What is a CA 125 blood test?
This test analyses the level of a protein called cancer antigen 125 in your blood. It is often used to keep an eye on certain cancers during and after treatment. For individuals at high risk of ovarian cancer, this test may assist in detecting early signs of the disease.
However, the CA 125 test is not necessarily reliable for use as a general screening tool for cancer of the ovary. Elevated levels of CA 125 can be caused by various conditions, many of which are non-cancerous, such as menstruation or uterine fibroids. Certain types of cancer, including peritoneal cancers, fallopian tubes, endometrial, peritoneal cancers, and ovarian can also raise the levels of CA 125.
Why is a CA 125 test necessary?
- Screening for high-risk individuals
A healthcare provider may advise you to undergo a CA 125 test if you have an increased genetic risk, such as BRCA1 or BRCA2 mutations, or a family history of ovarian cancer. Some experts suggest combining CA 125 tests with transvaginal ultrasounds every six to twelve months for high-risk individuals.
- Detecting cancer recurrence
Rising CA 125 levels after cancer treatment may indicate a recurrence. However, regular monitoring has not demonstrated improved outcomes and can result in additional, potentially unnecessary treatments.
- Monitoring cancer treatment
If you have been diagnosed with endometrial, ovarian, peritoneal, or fallopian tube cancer, your doctor may recommend regular CA 125 tests to track your condition and response to treatment.
While monitoring may detect cancer recurrence, it has not been proven to improve survival outcomes and can sometimes lead to unnecessary treatments like additional chemotherapy.
However, not all ovarian cancer patients exhibit elevated CA 125 levels. Screening may not reduce ovarian cancer mortality rates and may lead to unnecessary testing.
Test procedure
- Before the test
No special preparation is required, and there are no dietary restrictions before taking the test.
- During the test
The CA 125 blood test involves drawing a small blood sample from your arm. You may feel a minor sting or prick as the needle is inserted. The procedure is quick, typically taking only a few minutes.
- Collecting your results
Your healthcare provider will guide you on how to access your results, which are usually available within 24 hours. If your CA 125 levels are elevated, additional tests may be recommended to determine the cause.
What happens if CA 125 levels are high?
A high CA 125 level does not always mean cancer is present. Non-cancerous conditions, such as endometriosis or menstruation, can also cause elevated levels. If your CA 125 level is high, additional diagnostic tests, such as pelvic or transvaginal ultrasound, CT scans, or biopsies, may be required to confirm a diagnosis.
Receiving Your Results
Your healthcare provider will discuss how and when to access your results, which are typically available within 24 hours. If your CA 125 level is high, your medical practitioner will arrange further investigations to confirm the diagnosis.
If you have not received your results within the expected timeframe, contact your healthcare provider for clarification.
Understanding Your Results
- Elevated levels
Results exceeding 35 U/mL may indicate the presence of cancer cells or other medical conditions. For individuals without a history of ovarian cancer, further testing is required to determine the cause of elevated levels. For those who have had ovarian cancer, high CA 125 levels may suggest a recurrence.
- Normal range
CA 125 levels are typically measured in units per millilitre (U/mL). A normal result falls within the range of 0–35 U/mL.
